ARCHITECTURAL CONTROL COMMITTEE (ACC) MODIFICATION REQUEST FORM
The Homeowners' Declaration of Covenants, Restrictions and Easements for Smithwick Crossing require improvements, alterations, or additions to your home or property to be approved by the Smithwick Architectural Committee (ACC). This is done to maintain the standard and consistency for the homes in Smithwick Crossing. This approval is not a building permit. The ACC requires this application be filled out in its entirety, and (if applicable) a sketch or pictures, as well as a description of the proposed improvement. Please use the back of this sheet for additional specifications or information if necessary.
Please allow approximately two weeks for Committee Approval
Please review your covenants so that your request is in accordance.
In order to expedite your request, the ACC will need this application properly filled out, a sketch/pictures (if applicable) and a description of the proposed improvement. The sketch and description should be in triplicate so that an approved copy can be returned for your records, unless submitted via email at acc@smithwickcrossinghoa.org.

Please note that exterior improvements to your property require ACC approval. Additionally, you should obtain the approval of the local Building Department. The ACC is not authorized nor qualified to advise you on Building Department issues. We recommend that you consult with the Building Department if you have any questions regarding their requirements.
We strongly recommend that you employ licensed and fully insured Contractors when making improvements to your property.
Note: Work should be completed within 90 days of preliminary approval. Notify the Architectural Control Committee at when work is completed so a final inspection can be scheduled.
